Deine Aufgaben
- Du planst und koordinierst die individuellen Pflegemaßnahmen und dokumentierst den gesamten Prozess sorgfältig mit unserer IT-Software
- Außerdem übernimmst du die fachgerechte Grund- und Behandlungspflege nach ärztlicher Verordnung und stellst die Medikamentengabe sicher
- Als vertrauensvolle Ansprechperson berätst du unsere Klient:innen sowie deren Angehörige einfühlsam in allen pflegerischen Fragen
- Du achtest auf die Wünsche und Bedürfnisse unserer Klient:innen, um ihre Selbstständigkeit bestmöglich zu fördern und zu erhalten
- Im Team arbeitest du eng mit Ärzt:innen, Therapeut:innen und anderen Fachbereichen zusammen, um eine ganzheitliche Betreuung zu gewährleisten

Dein Profil
- Abgeschlossene Ausbildung als Pflegefachkraft, z. B. als Altenpfleger, Pflegefachfrau/Pflegefachmann oder Gesundheits- und Krankenpfleger (m/w/d)
- Idealerweise Berufserfahrung in der Kranken- und/oder Altenpflege
- Sicherer Umgang mit IT-gestützter Pflegedokumentation
- Geduld, Einfühlungsvermögen, Teamorientierung und eine ausgeglichene Persönlichkeit
